Ketones can be : Concentrated in the urine any time you go for a significant period of time fasting. The concentration depends upon your state of hydration and amount of urine production. Some people spill glucose into the urine without elevated blood sugar due to genetic defects. Most don't spill until glucose greater than 189-200.

Ketoacidisis: You may be approaching a critical condition diabetic ketoacidosis. Its when your blood sugar goes very high and you become dehydrated. Glucose and ketones re found in the urine. I would get checked by your MD today

U/A  glu pro ketones: Normally you should not  have glucose in urine,It is  a sign for diabetes .Would get a fasting blood glucose  and a hemoglobin A1C. Trace protein and ketones may be seen if dehydrated/fasting but  protein is also seen with diabetes  due to kidney issues (so may ketones but that is in severe diabetes). Discuss with your doctor/ urgent care

Few other causes: Uncontrolled diabetes and prolonged fasting = common causes, but hyperthyroidism, pregnancy, fever, inadequate carbohydrate intake such as those doing special diets like Atkins' diet.
Excessive exercise and dehydration may show some ketone on urine. 
Some drugs may also do this. Since you said you are chronically ill for 10+ years...you may want to consult doc for evaluation.

: Normal fasting blood glucose level is between 80-100mg/dl. But, a single high fasting glucose is not an accurate measure of prediabetes or diabetes. You Dr should follow up with a HgA1C which measures your blood glucose over time. HgA1C below 5.7% indicates normal blood glucose, Between 5.7% and 6.4% indicates prediabetes, and Over 6.4% indicates Diabetes.

: DKA is seen in Type I diabetes, and can be the catastrophic first sign.  T1DM is an autoimmune disease, but is found in children too young to receive Meningococcus vaccine and in children older.  Of the millions of children who are immunized on time, only a tiny fraction go on to develop autoimmune diseases; it would be unethical to test with children not receiving life saving vaccines.

Urine not diagnostic: Hi. The tests for diabetes (fasting glucose, post-challenge glucose, 3-month average glucose, and random glucose) all have their strengths and weaknesses. A1c is an excellent test and vital to the modern management of diabetes; it's NOT the most sensitive diagnostic test, however. Fasting and post-challenge glucoses can both reveal more subtle abnormalities. Random glucose is a blunt instrument.
