Ketones can be : Concentrated in the urine any time you go for a significant period of time fasting. The concentration depends upon your state of hydration and amount of urine production. Some people spill glucose into the urine without elevated blood sugar due to genetic defects. Most don't spill until glucose greater than 189-200.
