Niacin & Primrose : Niacin is a great solution for raising HDL naturally.  I typically recommend patients take time released Niacin to minimize the flush at a dose of 500mg three times per day.  Ask you doctor before going this route of course.

Another great remedy is to add evening primrose oil to your dietary supplements.

See doc.: Sounds like you need an internist.  BP seems excessively high for someone your age.  Need a good eval for secondary causes of htn.  Also if trig > 500 should consider treating.  If LDL also elevated atorva or rosuva statin might be good choice.  Congrats on wt loss.  Keep up the diet / exercise and wt loss.

Given : Given your husband's lack of obesity, lack of smoking, and non sedentary existence, i doubt that you have much to worry about.  Only if he has a strong family history of premature coronary disease would i be worried.  
Niacin can boost hdl.  Over the counter preparations are generally not as effective as prescription preparations and are subject to the same serious side effects as the prescription drugs.  Niacin is best used under the careful guidance of a physician.

Indirectly: Accutane incresases levels of a protein called apociii , which is an inhibtior of another protein apocii. Apo cii is the binding site for an important enzyme called lipoprotein lipase, which when acts on triglyceride containing particles removes or hydrolyzes triglycerides (tg's). Too much apociii (via accutane) raises tg's. High tg's are a very common cause of low hdl. Thus Accutane lowers hdl.

Niacin does so a bit: Niacin =nicotinic acid, commonly known as vitamin b3, is available otc as a supplement. Also availabe via prescription with a much higher dosage and is used to treat hyperlipidemia. Comparing to other cholesterol pills, Niacin is actually good in raising hdl, but can also cause higher sugar and liver abnormality. A flushing-side effect is bothersome for many. Consult doc.
