are there potentially dangerous side effects of long-term usage of hydrochlorothiazide (12.5 mg) daily for blood pressure?  i have been taking one capsule daily for around 2 years now.  thank you?

: Hydrochlorothiazide is a commonly prescribed diuretic (water pill) medication for high blood pressure.  It is generally considered safe to use long term.  It would be important to have routine monitoring of your kidney function and electrolytes, particularly sodium and potassium while on this medication.

Diuresis: If you truly have fluid retention you could ask your physician if you could increase the dose to 50 mgs as needed or switch to a more potent diuretic like furosemide. Also ask about taking a potassium supplement with the diuretic. Do not increase your dose of hctz without your physicians OK.

Different diuretics : Lasix  (l) and Bumex (b) are diuretics that work at the loop of henle n the kidney and are more portent that hctz (hydrochlorothiazide). All diuretics will may you lose potassium, sodium and magnesium. B and l are stronger than hctz (hydrochlorothiazide) and can be used in patients with chronic kidney disease where hctz (hydrochlorothiazide) will be ineffective. All diuretics will increase your urine output. B and l do not have any effect on glucose levels.

There : There are many diuretics which work on different parts of the kidney to effectively help you "lose salt (sodium)" and depending on where in the kidney they work, they can have differing side effects. Some can affect calcium more, some can cause you to lose potassium more than others (i.e. Furosemide is a "loop" diuretic which causes more loss of potassium, vs. Spirinolactone which is a "potassium sparing" diuretic), and so on. I would recommend talking with your physician about your side effects and which alternatives might work for you.

I would tell your physician (P) about your problem and see if the H can be changed for another BP medicine. I would also ask him to consider you to have secondary hypertension. Having essential HBP at 21 years of age is most unusual.

is stopping hydrochlorothiazide suddenly dangerous? my other blood pressure pills warn to not stop them suddenly or there could be bad side effects. is it the same for hydrochlorothiazide?

I : I don't think that stopping it suddenly is dangerous.  Other medications such as beta blockers should not be discontinued immediately if you have been on them for a long period of time due to potential rebound from beta blocker withdrawal that can lead to elevated blood pressures and heart rates.  Other medications can also sometimes cause rebound hypertension with rapid withdrawal, but a diuretic such as hctz (hydrochlorothiazide) should not be dangerous.

maybe hctz, (hydrochlorothiazide) as a hypertensive, has fewer side effects, i.e., dizziness? if so, will ask md to move from 25 mg to 50 mg next week. worthwhile?

No : At 50 mg you don't much more more blood pressure lowering effects and increase risk of side effects. With it comes to hypertension therapy, using two medications with differing mechanisms of action is suggested than increasing the dose of a single medication.
