It is impossible to give you an accurate guess without a lot more information, incl. how long you have felt this way, your medical history, and list of medications. : The combination of unilateral tinnitus, hearing loss and vertigo is a reason to check for the presence of an acoustic neuronoma,a brain growth. I'm not sure if this fits your profile, but I feel that a consultation with a neurologist or otolaryngologist is warranted.

Local or systemic: Could be related to local disease process like sinus or nasal infection or more a more systemic process such as flu or other infection, dehydration etc.  Imuran would predispose you to infections.

Complex issue: Tinnitus is just the finding of ringing in the ears.  It can be the result of damage to the inner ear or due to development of a tumor of the ear nerve.  Sound trauma and head trauma can damage the inner ear and ear nerve and cause the complex of symptoms that you describe.  You need a thorough evaluation with attention to the inner ear.
