had a small pink papule on my back. derm looked at it with dermascopy and said it looked like a normal inflamed mole. biopsy came back as a lichen planus keratosis. does this make sense even though dermascope said it was a mole?

Yes: histological bx is the major determining factor. question is...if dermoscopy revealed a benign lesion, it's curious that it was  even biopsied and not just followed up on subsequent visits?

Several: Lichen planus often requires no biopsy. A topical glucocorticoid to alter the immune dysfunction that causes the pathology, or pricey tacrolimus. A retinoic acid derivative may also help. Lidocaine mouthwash for oral pain if it is there. Be checked for hepatitis c, know what chemical you meet at work and what rx's you take if applicable.

Not immediate: Treatment for lichen planus varies with the individual.  There are generally triggers that set off some of the signs and symptoms.  Why does it happen?  It attacks numerous spots on the body.  If you are looking for a good source of information, research Baylor University.  They have a clinic treating lichen planus.  The have a online support group for info and help.

Treatment: No known cause or treatment for lp. We treat the symptoms so see your dermatologist/oral surgeon if in the mouth. It may resolve on its own or may continue to wax and wane.

See a doctor. : Most cases of lichen planus are relatively mild. Affected individuals who do not have symptoms do not need treatment. 
If the itch or appearance of the rash are unpleasant, topical corticosteroid creams may be applied. In some cases for localized, itchy, thick lesions, injections of corticosteroids may be given. You need to see a doctor for proper diagnosis and treatment.

i have severe skin only lichen planus, was given oral prednisone 20mg for 9 days. what are chances planus will return more severe after treatment?

If diagnosis is-: If you have lichen plants, a brief corse of Prednisone will usually not prevent recurrence! that said, i believe a skin biopsy is necessary. A visual diagnosis can be misleading! best!

Cortisone cream: Corticosteroids are usually prescribed for lp. There are some other treatments depending on the severity, but that would be at your doctors discretion. Lp is an immune system response and is usually self limiting, meaning it should resolve in 8 to 12 months.
