i developed a pars fracture one year after an endoscopic laminotomy for a l4-5 issue on the left side.   i am now having right leg tingling and cramping. medial nerve block did not work. epidural worked for 2 months.   what will happen next?

Low back pain : Your answer is very specific to your situation. It is difficult to properly answer this without getting a full picture. Sometimes people have relief with a set of three epidurals,  some are better with physical therapy. There is also integrative medicine such as massage, acupuncture.  Opioids have much more side effects than benefits for most people with back pain.

Depends on type: Treatment depends on type of fracture, stability of fracture, how much deformity fracture introduces to the spine. It also depends on how much spinal canal compromise, and if there is any neurological deficit present.

Most fractures of L4 and L5 vertebral bodies can be treated non-operatively with bracing, activity modification. A few may require surgery. Your surgeon will let you know.

Vertebral fracture: Optimally - wear spine brace (padded plastic, front and back) for minimum 2 months, 3 months best.  Wear morning til night.  Can sleep without.  Minimal walking x two weeks.  Only sit or recline.  Cannot drive or bike.  No lifting x 3 months, no sexual activity x 3 months, no domestic chores, no twisting, bending other than sitting and standing.  Walk gently, slowly.

Worry: Your symptoms indicate that you likely have a radiculopathy, which means a nerve root to your leg is not functioning correctly.  This could be caused by a herniated disc caused by your injury, a piece of bone (either a piece of broken bone or a bone spur) or even a shift of the bones you injured (called spondylolisthesis) with respect to one another.  See a board certified spine specialist.

Not worrisome: probably not a worry unless its below lamina and may start to compress the collection of nerve rootlets below it over time - sounds like you had a trauma in the past, there was a small fracture of the spinous process (which in and of itself might be painful for a few weeks but likely will heal and come to nothing)  and now the healing is leading to some bone formation in the vicinity

Lumbar. Fracture: Treatment depends upon whether the fracture is stable or not and amount of compromise of The spinal canal or neuroforamen. Expect a thorough physical exam ,ct scan and MRI. If you already have some neuro compromise there may be use of medication to lessen the ongoing damage . Of course brace and activity limitation is critical

Hopefully: After healing, the bone should get back to normal strength. Recovery from a spine fracture and back surgery depends on the degree of injury, your own healing process, determination in rehabilitation, luck, and what type of work and activity you are trying to get back to!! I have patients who have returned to full normal activity in physically demanding jobs, but others were not able to. Good luck!

i had spondylolisthesis 4 times. i had a total of 7 fractures in my l5 and l4. i am 24 and have been told i am too young for fusion. what can i do?

Is it symptomatic?: painless spondylolithesis if low grade can often be managed non operatively but if it is progressive and has neurological symptoms or has pain that fails conservative care surgery can be considered up to and including fusion
