: The first Doctor to see should be :Neurologist,then  by Consultation with Orthopaedic  Surgeon.The reason is: you need to have a precise diagnosis of your spine problem to adjust the right treatment just for you.There is no indication for any manipulation or directed exercises  prior to a correct diagnosis …for an appropriate treatment.

what is acupotomy done by an acupunture doctor? is it a safe non invasive procedure & if so. would it help stenosis l3-4 & l4-5?

: Since a scalpel is also used in addition to the acupuncture needles,I would really have to consider acuptomy a combination of acupuncture and surgery. If there's a lot of cutting involved, it's predominantly surgery and it's difficult to know how much acupuncture itself contributes to the procedure. For spinal stenosis,I favor traditional neurosurgery if I were the patient.

Yes: Both a disc hernaition at l2-3 and lumbar stenosis from L1 to L4 can cause pain in the back of the thighs. A significant disc herniation may cause lumbar stenosis.

Mild Back arthritis: Degenerative changes of spine all above findings are 30% asymptomatic may or not be cause of your symptoms .Now you have a more detailed list of suspects to investigate with a spine specialist.

Depends:  : Can be very problematic with leg and back pain requiring treatment.  It is almost normal for all human beings to have some l5/s1 and l4/l5 facet arthritis and disc disease.  Rarely requires treatment more than core exercises, nsaids, back rehab and other conservative/ fitness training.

both my knees have a partial bone-on-bone. i also have severe stenosis on l2 l3 & l4 l5. my legs ache. do u think it's from knees or from the severe stenosis causing the bad ache in both my legs?

: Most likely a combination with the spinal stenosis likely involved in pain affecting both legs, including thighs, with the knee arthritis adding pain bilateral knees and below. An ortho eval may help clarify.
