Suspicious: You should be guided by an endocrinologist as to further testing which will include a fna biopsy. Sometimes it is just best to remove a solid nodule of this large size.

Yes: Yes image guided fine needle aspiration cytology will give diagnosis , a low risk out patient procedure.

Probably benign: Depends on the appearance on ultrasound and also on lab and clinical parameters. Usually don't biopsy this size lesion without some other indication that there's a problem, but it should definitely be followed-up over time with repeat exams.

Hmm.: The newer affirma test should bring your risk of thyroid cancer down to 5%, but this is a larger nodule with vascularity and it isn't unreasonable to get it out.  It would be definitive for diagnosis, especially if you don't want to follow it long term.  Totally up to you and your c.

Depends: Thyroid nodules are very common.  About 70% of the population has a nodule (many don't know it).  About 5% of nodules are cancerous, which is why we care. Unless there were concerning characteristics on the ultrasound like microcalcifications, taller than wide or increased blood flow or you have a family history or radiation exposure, it may be ok to follow-up with annual ultrasounds.

: Calculating probability of cancer can be done but we (and your Drs) need to know your family history, if you grew up/live in an endemic goiter part of world, are eating enough iodine, past biopsy results if any results of your radioactive scan.  Get back when you know these things. You are young to have ca unless it runs in your family.  Do thyroid family tree.  Apple does not fall far from tree.

Thyroid nodule: Thyroid cysts can certainly come/go.  The quality of ultrasounds, the interpretation, and the definition of a "nodule" can vary.  Hashimoto's can cause a patchy pattern that can look like a nodule. Certainly a small nodule that is so transient is not something to worry about.  An endocrinologist should be able to interpret things.

Variable: Thyroid nodules are variable in their growth patterns.  Some can grow quickly in size, particularly if there is a cystic component to the nodule.  Your doctor will usually suggest an ultrasound to follow the growth of the nodule every 6 months to one year, depending on the level of suspicion.

You can't : Thyroid nodules don't shrink, and there is no medicine or other treatment that will shrink them short of surgical removal.
