Too much estrogen: If the endometrium is abnormally thickened then estrogen is the usual source for the same. It can be from hormone replacement therapy unopposed by progesterone or large amounts of natural estrogen from the ovaries or elsewhere.

A little thick: Individual cases must be evaluated by a doctor. In general we expect the endometrial lining to be less than 5 mm in a post menopausal woman. Your obgyn can determine based on your unique history if an endometrial biopsy or other evaluation is warranted or not.

Overgrowth: After menopause the endometrial lining should be thin ideally less than 5mm on a sonogram. If it is above 25 mm this is concerning. Measurements in between must be analyzed on an individual basis. Causes include fibroids, polyps, endometrial hyperplasia and in some cases endometrial cancer

Not always: Not always, but certainly would be suspicious for cancer-and further evaluation- i.e. Biopsy should be done to rule in/rule out. 
Biopsy is the only thing that can tell you for sure whether it is malignancy or not.

: "Post menopausal bleeding" at "45 years of age" is unusual, as the average age for menopause is 54. Bleeding with chronic cervicitis is activated by sexual activity, and if no dysplasia is found on Pap smears, it is easily treated with cryotherapy.  See your GYN for guidance.

Bleed: If post menopausal lining should be less than 5 mm. If you are having abnormal bleeding i would do the biopsy just to be safe.

Estrogen stimulation: It means that there is a source of new estrogen stimulation that is causing the dormant endometrium to proliferate again. Her MD needs to document an estrogen source (medications or natural body) to prevent continuation of this process which can lead to endometrial cancer in a post menopausal woman.

impression: 1) heterogeneous myometrial echotexture with ill defined hypoechoic areas 2) focally thickened endometrium upto 8 mm for post menopausal status. what do these 2 impressions mean? thank you ?

Accepted Answer

: Heterogeneous myometrium may represent fibroids or possibly adenomyosis, neither of which are a concern postmenopausal. The 8 mm endometrium is not normal and should be further evaluated or at least closely monitored. It would be particularly worrisome if you have had some bleeding, even small amounts.
