Clozapram switch: If you have been on clozapram and ciltalopram and switched to Paxil (paroxetine) with no break, this could be a problems considering how long htis clienthas been on the current meds. Clozapram is a major tranquilizer used mainly for those with severe symptoms of schizophrenia and possbily a severe depession. Paxil (paroxetine) is an antidepressant and may help, but consider another consultant if you  are uncomfortable.

B12: Paxil (paroxetine) use in and of itself should not require B12 vitamiins.   B12 is generally available in  green leafy vegetables.  Unless you are elderly or have a gastric issue you should be able to absorb the B12 in a healthy diet.  Too much B12  can give you neurological issues. Too much is not any better than too little.

Sounds good, but: Why not get a formal reduction plan from thje doctor who prescribed? In my thinking, it's part of their responsibility to you as their patient. You do seem to have a proper plan, but if you notice feelings you don't like - get help, don't try to address them by ups and downs on your own. It hope it goes well.

Yes bit unlikely: An overdose with antidepressants like paroxetine alone is usually not fatal. However,  mixed with alcohol or other sedating drugs things can get bad in a hurry and you can die. Also if it is a child or an elderly person or someone who is medically ill and you take a large amount of paroxetine then death is a possible outcome. Keep all meds in a safe place & take only as directed by your dr.
