You : You are likely dealing with a congenital problem (meaning you were born with it) called pectus excavatum. This is a condition in which a the breastbone is sunken into the chest, giving it a caved in appearance. There are varying degress of this. Your doctor may wish to an an x-ray or other imaging studies in order to assure there is no damage or impending damage to your organs. If so, surgery may be recommended. 
As far as your breasts are concerned, small breasts are not related and are based on genetics. Also one breast being bigger than the other is a common occurrence . Surgery is also an option.

Normal: It can be normal if you have breast stimulation despite not breast feeding, but you doctor can check your hormone levels.

If : If a patient has spotting and the symptoms described, and there is any possibility of pregnancy, then a pregnancy test should be checked.  Sometimes other hormonal fluctuations (from physical stress on the body, weight (too high or too low)) can cause abnormal spotting.  Perimenopausal patients get spotting at times though 41 would be considered very young for that.   If spotting continued of the patient had other symptoms, then the patient should see their provider for an evaluation, and also to be sure that their pap is up to date.

Yes: People get hairs on and near their nipples. People are also free to yank the hairs out, if bothered by the hairs. They usually grow back again, and have to be plucked again. If a girl or woman thinks she is getting too much body hair, compared to her friends and classmates, she can see her primary care doctor to make sure all is well.

Yes.Even men can get: Breast cancer.Follow regular guidelines with regard to cancer screening and if you have found a suspicious lesion, get checked out.

It can be...: Without any other symptoms or information, this is likely normal. Body fat distribution (not just total mass) is determined by a very complex interplay between hormones that are very actively changing before, during and after puberty. So as the body changes/matures, the body fat (aka adipose) distribution may also change. If you're worried, though, go see your doctor, they can help.

Not certain cancer.: The left and right sides of our bodies are never perfectly symmetrical so the fact that the breasts are not mirror images of each other is not concerning. However, if you're saying that one of your breasts looks different than it usually would look, then you would really need an investigation including an exam and mammogram at least.

Hormone variation: can increase breast tenderness,  for example if your cycles are irregular,  polycystic ovaries, can increase levels of weak estrogen, this could be a cause, hormonal contraception could also be related to breast pain, in some cases. If you feel a mass, or pain persists, discuss with your doctor.

Normal: Hormone stimulation can cause the beast buds to be sensitive. Time for a talk about her periods because that will follow.
