This is best determined by a physician who can distinguish the source and prescribe properly.

Removal: Careful removal of the tick, followed, if necessary, by a steroid cream or ointment to reduce the inflammation and itching. Would not recommend prophylactic antibiotics.

Stay indoors :): Seriously, the best way is to not go where ticks live.  The next best strategies are to wear appropriate clothing...Long pants, long sleeves, hat, and stay on the trails instead of blazing through the brush.  You might also consider treating your clothing with a repellent prior to hiking.  When you return, immediately launder your clothes, do a visual inspection, and check the dog too.
