Not likely: What you describe in a neck fold sounds like a candidal yeast infection, which would respond to an antifungal like nystatin

dermatologist says my infant son has developed a congenital nevus (specifically a cafe au lait spot) on his face within past few weeks. how likely will this turn into a huge, black, hairy mole? nervous! :-(

Probably low: The cafe au last probably will not become a hairy nevus but should keep an eye out for any new cafe lesions.

Good news is they are laser able however the return frequency is high. If you go that route make sure to go to a dermatologist that performs a lot of these type treatments and start on a small area and wait 6 weeks to see if the spot lightens or darkens.

my infant son was sent home from daycare due to them thinking he has ringworm on his face. he has eczema and has been using a pacifier that has been rubbing the spot where the ringworm is. it did get slightly better with eczema cream. is it ringworm?

Few thoughts...: A few things that come to mind include: atrophy of muscle in the area, a reaction from a steroid injection or topical steroid, fibrosis of the area maybe after some a trauma... Or it can just be a normal variation of your body!  if it's developed over a period of weeks to months, it's something to have checked by your doc - not urgently, but sometime in the near future. Probably ok but...: Most parents don't notice the soft spot as being sunken, because there is rarely a reason to check it at home. Since the spot is not filled in with bone yet, its surface is normally below the height of the surrounding bone, so is always sunken from that perspective. We worry more only in sick babies, if the spot feels sunken a lot. We also worry if it feels swollen in a calm, resting baby.

Probably. : It's normal for the soft spot to flatten, sink and bulge depending on positioning, crying and hydration status.  And infants do normally have visible scalp veins.  What matters is how is the baby acting?  Feeding ok? Sleeping ok? Irritable? Fever? If other issues, contact your pediatrician.

Not much: If a child is otherwise well, taking normal feeds and urinating routinely without vomiting, diarrhea, fevers, etc, a slight depression of the fontanelle ("soft spot") need not be cause for alarm. Unless something else of concern is happening, this is not a reason to visit the ER. A pediatrician could safely confirm that this is normal during regular hours.
