if i accidentally took 2 20mg tabs of xarelto, should i go to a hospital or my doctor? i feel fine. they are small and for some reason i am thinking2

Not necessarily: Why are you taking Xarelto? You don't need to do anything unless you have excessive bleeding or bruising or any new symptoms. Xarelto is metabolized within 24 hrs, so it's safe to resume treatment with one 20 mg tablet daily the next day. In the future take your Xarelto at the same time each day, and mark it on your calendar.

Xarelto is most: effective when taken with a meal. It is recommended that for any dose over 15mg it be taken with supper to assure it is with a meal. Once you refill his medication he should take his usual dose daily with supper. Don't take two doses to make up! Try not to miss doses in the future. It only works if he is taking it regularly. Look in the bottle to know ahead of time and to avoid running out.

Don't take any today: 20mg is the maximum dose.  Safer overall to take your usual dose tomorrow than inadvertently take extra

Niacin &amp; insomnia: Interesting! I had never heard of taking niacin for insomnia but I found many websites discussing just this use, albeit none in peer-reviewed journals. Regardless, some forms of niacin may lead to liver issues while others can cause tremendous flushing &amp; itching. Good news is that there's no interaction btwn Xarelto &amp; Niacin (http://goo.gl/B1QYXD). Still you should always tell your doc everything

: Don’t take 2 tablets. If you forget one day , most likely will be no harm . But if you take double dose - too high risk of bleeding . 
If you need surgery for example , Xarelto needs to be stopped for few days . It’s taking risk between bleeding and other medical conditions. Take your next pill in next scheduled time . Try to make pillbox with days on it - so it’s easier to see if you took it

Take your xarelto: An extra dose of xarelto will likely not cause any increased risk of bleeding. Missing a dose places you at risk of not being anticoagulated. You should discuss with the prescriber

Tablet or the dose?: Xarelto affects blood clotting &amp; shouldn't be stopped abruptly. 15 mg or 20 mg tablets may be crushed and mixed with applesauce immediately prior to swallowing. After swallowing, the dose should be immediately followed by food.
