Uterine Perforation: It can happen but rarely if appropriate measures are taken. Ie - knowing the position, size, and shape of the uterus before inserting instruments (exam +/- ultrasound). Experienced gyn surgeons rarely perf a uterus and if it does occur, they can recognize it immediately. Midline perfs usually heal well, lateral trauma can damage blood vessels & need repair. So, it's  uncommon but possible.

Cramping/ gas?: If there is pain to the shoulder / upper abdomen this could be associated with gas introduced into the area during the hysteroscopy.  Post operative cramping can also occur.

Overall very safe: A diagnostic hysteroscopy carries a small risk of uterine perforation, bleeding or infection.  Operative hysteroscopy for removal of fibroids or an abnormal uterine septum(wall) carries a higher risk of perforation.

Hysterosalpingogram: A hysteroscopy would allow your doctor to determine whether there was something within the uterine cavity that may be a factor in keeping a pregnancy but wouldn't let you know about fallopian tubes.  If you had a laparoscopy at the same time, then you would know. Alternatively, you could have a hysterosalpingogram that looks at both the uterine cavity and the fallopian tubes.

Hysteroscopy: You ask a question that is difficult to answer with only the information provided. Mainly, why would a 23 yo be needing a hysteroscopy and what was done at that time.  Also, are you tracking ovulation or did you just miss your period. In this instance your best asking this to the GYN that did your surgery.

Yes: Hysteroscopy is simply a 'look' inside the uterus with a scope. Hysterectomy is the removal of the uterus and pregnancy then is only possible with assisted means such as in vitro fertilization(ivf).

Hysteroscopy: It is difficult to answer this without knowing the details of the procedure. If it was just a diagnostic procedure to make sure everything was alright, then the procedure would not make it more difficult to become pregnancy.
