Be sure the pediatri: Might be a depigmented nevus: no concern. If an ash leaf lesion it could be the first sign of tuberous sclerosis. Be sure your pediatrician reviews what to look for. The tuberous sclerosis society can supply you with literature which you can share with your dr. If there are suspicians, take the baby to an academic center known to be doing studies on this. Most drs have no experience or knowledge o.

Ideas: This is often an age-related change on the shins and forearms of middle aged folks. It's not really treatable and not dangerous. If it comes and goes, rule out tines versicolor. If it keeps popping up in new places, consider vitiligo.

How raised is it: A macule is flat. Can't feel it with your eyes closed. A papule is raised and can be felt. A vesicle is a papule that has fluid in it - like a tear drop. It may also have pus in it- yellowish - then it's called a pustule. All these lesions are smaller than 5 mm in greatest dimension. Please google for images.

Biopsy needed: The only way to know for certain if those melanotic areas are actually benign is with a biopsy. I encourage you to go to a medical clinic and have an exam. Discuss your concerns and ask about a biopsy of the lesions. The clinic doctor may than refer you to a dermatologist. Best of luck with your health!

No: Skinoren (azelaic acid) will help with acne, but if your spots are not due to acne, it won't help.  If your spots are recent, it may be due to a new product you are using on your face and you should stop it and see if the spots disappear. If they don't, get seen to find out what's causing them.

Micro needling: "is micro needling combined with sun exposure effective for treating hypopigmented scars?"  micro needling is a new procedure that is supposed to stimulate collagen growth in treated areas and probably won't help for hypopigmented spots. Sun exposure has been a common treatment for that for years though and may help you.

Let them fade: I do see that you are being treated for seborrhea with ketoconazole, and also for acne Clindamycin monotherapy. It's a serious mistake to treat acne just with an antibiotic without topical benzoyl peroxide or some similar medication. Better management of your active acne will make the question of red and dark spots after acne moot. Good luck.
