Chemotherapy: it is a chemotherapy drug (a mild one, that doesn't have severe side effects, once it has been checked, worries about it lowering blood counts too much are not much).

It does lower sperm quality and that effect can last after it is stopped.

Unknown: No the baba HUG trial demosntrated clinical improvment to patients with primarly sickle cell disease (but also thalasemia) with the use of hydrea (hydroxyurea).  There have been no reproted long term consequences of hydrea (hydroxyurea) therapy in young children.  Hydrea (hydroxyurea) is a teratogen and should not be given to pregnant women, but thus far has not been reported as a cause female infertility, only male.

Yes: Hydre increases a type of hemoglobin called fetal hemoglobin in sicle cell patients and this prevents the sickle cells from breaking up easily. Hydrea (hydroxyurea) can reduce need for transfusions and patients may have less pain crises, chest sydrome and other complications.

As long as it works: This medication has been studied for a long time.  There has been no reported cases of secondary cancers directly related to hydrea (hydroxyurea).  It just needs to be monitored, as you know.  Hope this helps.

I say no: The talk of an increased leukemia risk has been with us since the 1970s without anybody being able to measure it. If it's real, it's tiny.  Anagrelide isn't under suspicion. If some overzealous practitioner went after a high platelet count without good reason, I'm sorry but you'll be fine. Why do you ask?

if a plt is normal but starts to fall (even getting down to 8) & this is suspected to be a drug effect (hydroxyurea) & the drug is ceased & the low plt resolves itself within 7 days then would an undetected infection stop the plt from normalising?

Likely not: Not sure what you mean by undetected? meaning asymptomatic or not tested for?  There are viruses that can cause decreased marrow production (Parvovirus) but the usually case symptoms. Low platelet counts do not necessarily predispose you to infections but the count  can change, up or down, with infection.

Unclear: I dont see any reliable information on using hydroxyurea under the age of 2 for decreasing transfusion needs in beta thalassemia babies.

1 month old : Homozygous hemoglobin E disease (genetic; inherited from both parents) gives mild beta thalassemia &amp; anemia &amp; mildly enlarged spleen; probably doesn't need treatment.  Hgb E beta thalassemia disease (E from 1 parent, beta thal from the other): severe incurable disease. Hydroxyurea safe in infants. My guess: start early as possible after 1 mo old. Do discuss this with your pediatric hematologist.
