burn on thumb from hot steam.   very red.  looks patchy or maybe "future blisters" (hard to tell)   internet keeps saying hand burns need to be treated differently.  what is best treatment and does being on hand change that?   pain is decreasing.  thanks

Size matters here: Any burn's seriousness &amp; urgency of treatment depends on how big the wound is, what caused it, and how deep.  Typically steam from a tea kettle or hot food will be first or second degree. The area, if small and getting less painful and is not surrounding the whole finger or a joint can be treated like any other minor burn.  Silvadene (silver sulfadiazine) helps the pain (but is a prescription). Keep clean, protect area.

How serious?: This can be very serious depending on whether it's 1st, 2nd or 3rd degree and what extent of area is burned. You absolutely should see your doctor unless it's trivial. If trivial, put antibiotic ointment on it, a telfa dressing, and analgesics for pain.

Ice for 1st 2 days: In general, ice will help decrease swelling and pain for the first 2 days.  Topical antibiotic ointment will prevent infection if blistering occurs.  If just red like a sunburn, don't worry.  If you have no sensation of the burn area, it may be a deep burn (deep enough to damage nerve endings).  If so, get to see your doctor to make sure you don't need more aggressive care than above.

Frostbite: Since they are 2 days old, they are probably already rewarmed. Treat the area like a thermal burn. Keep it covered with dry sterile dressings. If blisters develop, seek medical attention. Patience: You have done the basics. Now you sit back and monitor the site at least 2X/d while looking for signs of increasing redness or drainage that would suggest a new germ invasion. Most of these take about two weeks to dry/heal primarily and 6 weeks to look pretty normal. I leave the bubbles alone and they shrink/ dry to the skin and later flake off.

1st degree burns: First degree burns involve only the first layer of skin, and usually appear red (like sunburn).  Blistering does not generally occur with this type, although some peeling can  appear during healing.  Treatment consists of cooling the burn with cool water, and keeping covered with a non-adherent dressing.  OTC pain meds may be useful.  First degree burns usually heal well without scarring.

Possibilities: The possibilities include various infections like impetigo that will require prescription medication. There are other options, I have seen chickenpox look this way too, but with impetigo on top of the cpx.
