Multiple causes: Most women have periods lasting for 3-7 days but for some the bleeding can be longer than a week. Hormonal imbalances such as hypothyroidism, obesity, or structural issues arising from fibroids are common culprits that cause prolonged bleeding. 
if you have heavy bleeding using &gt; 2 pads / hr ; talk to your GYN for further options.

my menstrual cycle shifted from being regular to irregular for the past 2 years. for the past 2 months, my menstrual bleed has lengthened to 2-3 weeks?

Reason for GYN MD: Period changes such as yours is a good reason to see a GYN MD since this has lasted for at least two years. There are many causes for such irregularity, too many to list here.

is it normal to all of a sudden experience an extended menstrual cycle when haven't had one in years since getting mirena (levonorgestrel)?

Not normal: This doesn't happen to most patients with mirena (levonorgestrel).  It could indicate breakthrough bleeding due to the progestin hormone in the iud, could be due to an ovarian cyst, or - possibly due to a pregnancy (that is rare, but possible).  Should definitely check a pregnancy test, and see doctor if it persists.

Common problem: Irregular periods are one of the most common problems obgyns treat. Polyps, fibroids, infections, hormonal imbalances and many other causes are possible.  If this is a recurring problem you should be evaluated.   There are many options available to help regulate the menstrual cycle.  Schedule a visit.

See details: Assuming that you are not pregnant, d&amp;c's rarely provide long  term correction of bleeding problems. They are helpful at stopping bleeding quickly and diagnosing some endometrial disorders, but they will not correct bleeding from hormonal disorders or structural causes such as polyps and fibroids.  I would recommend additional evaluation if a specific cause has not been determined.

Don't delay: I would see your gynecologist as soon as possible since it is unclear how much blood you were losing and that might explain your dizziness.

: Menorrhagia (vaginal bleeding beyond 7 days) is less likely caused by fever and a couple days of using antipyretic agent. You can observe for now but if menorrhagia recurs next month or if you have bleeding again before the next cycle then you need to see a GYN doctor for ruling out structural or hormonal abnormality.
