Who is primarily in charge of your medical decisions? Yourself or others? Do you carefully pay attention to results of suggestions others make/promote and how you possibly accept/implement them? I would suggest doing so.

No: No, you do not have to take it at regular intervals, around breakfast, lunch and dinner will suffice.
6:00, 14:00, 22:00 will work, too, and that will be at regular intervals without getting up in the middle of the night.

Xifaxan (rifaximin) NOT for IBS: Ibs is a functional gastrointestinal disorder characterized by pain/cramping, altered bowel habits in the absence of specific/unique organic pathology/disease. Other symptoms common to ibs include bloating, gassiness, frequent/urge to have bm, early satiety, dyspepsia etc...
Often = nuisances, but can be debilitating at times.. Antibiotics don't treat ibs.

Depends on what you: are on it for.  It is commonly used in people with very severe liver disease, and if this is what you are on it for, then the answer is a definite NO.  Not because any interactions with the xifaxan, (rifaximin) but because alcohol in ANY amount is absolutely contraindicated and extremely dangerous if you have liver damage. You need to be asking your doctor these questions, with your whole health history.

i am going to be taking neomycin and xifaxan (rifaximin) 2x a day to get rid of my sibo. should i take them both at the same time or spaced apart?

Drug allergy: Isn't generic, meaning if you're allergic to one drug then u r allergic to all others. Also drug allergy is often confused with adverse drug reactions,so a patient can't tell if it is a true drug allergy or just an adverse reaction. If you are allergic to xifaxan, (rifaximin) other antibiotics are there to use. Also ask your doctor about probiotics, and elemental diet to treat your condition, best wishes

Possibly: Can cause nausea and upset stomach so i suppose that could lead to decreased appetite.
