Cold: Need to see your physician. Cold that does not get better may be complicated by a secondary bacterial infection like sinus infection/ ear infection/ tonsillitis.

Eustacian tubes: The tube that runs from your middle ear to the back of the throat, if obstructed by swelling due to inflammation will produce ear pain.  You may want to try a decongestant or a saline irrigation of the nares with a neti-pot.  Try these and if still in pain go back and see your doctor.

my 5 year old had a really bad ear ache. he has had a cold so this followed. i treated the symptoms and he finally slept a little last night. he woke up this morning with the ear ache but not as bad. he wants to sleep and is not hungry. no fever...

Should be seen.: A diagnosis of Otitis media can only be made by visualizing the ear drum. So that is my best advice, before it progresses.

: Tinnitus or transient hearing impairment may occur after the ear is exposed to a unusually loud noise but earache is uncommon, especially just a yelling. It could be coincidental. You should have your ear checked if pain persists the next day. You can take Ibuprofen OTC 200 mg x 2 or 3 tablets q 6 hours as needed for a couple doses.

BPPV: The fact that the spinning lasts for seconds and is probably related to position sounds like benign paroxysmal positional vertigo (bppv).  This is a disorder of the inner ear wherein loose calcium carbonate stones move and cause the inner ear balance mechanism to be activated.  Time, head exercises, or vestibular rehab by a physical therapist will help.

Could be but...: more likely a blocked eustacian tube in the neck. This tube is designed to eqalize pressure in the middle ear. When blocked it will produce an earache. Try using decongestants. If it is not relieved see your PCP of evaluation.

Earache treatment: At home remedies include warm (not hot) mineral oil for soothing, over-the-counter Ibuprofen (2-3 tablets up to 3 times a day, depending on the child's size). An additional home treatment can be purchased otc --benzocaine drops (e.g., auralgan), a numbing agent. In most cases antibiotics aren't necessary. Currently many experts recommend withholding antibiotics for 48 hours to see if it can heal.

It might be worth doing so to make sure that there is no infection causing your symptoms
