Dose adjust: I would expect to see some effect at 4 weeks. It may mean the Zoloft (sertraline) dose is too low for you.

i need to find ssri that will work for severe  anxiety. paxil (paroxetine) made me gain lbs,lexapro and zoloft stopped working . would celexa work. others?

I'll say it again: I think I've already responded to you, but that's okay. There may be an SSRI or SSNRI that will work for you w/out bad side effects, but it gets slimmer each time you try one and fail. Definitely have medical guidance as you sample them - it can be risky to stop and start over and over. Like I say to most... Consider good fit CBT therapy. Find one you trust and work at it. Best!

SSRI's failed: So one SSRI may work when another does not. Keep trying. There is a new one called Brintillex which has much greater effectiveness in the reported studies. 
Also a Selegeline patch may prove effective.

Try counseling : While some medications will not work there are meds that will but along with counseling as well.  Anxiety and depression are best treated with both behavior modification and medication.  Give it some thought and discuss with your provider.

Any time.: One nice thing about zoloft (sertraline) is that you can take it any time of the day. If you have routine built in (such as right before bed, first thing in the morning, with dinner, etc.) it will be easier to remember to take it.

Medicine changes : it will be fine to transition from one medication to another with an overlap time.    You can overlap by removing the Zoloft over a two week time frame while adding the Lexapro (escitalopram) gradually or all at once.   It takes several days for the Lexapro (escitalopram) to gear up in the body.  Monitor him for tiredness, mood changes and sleep changes.   These will go away after the total switch.

Can just switch: Many psychiatrists and others will taper one of the SSRIs down while increasing the dose of the other, in order to avoid the discontinuation syndrome and make sure that the other drug isn't producing more adverse drug reactions.  However, I have simply switched people over without any significant problems. It can be done either way. Listen to your doctor and/or pharmacist for advice on this.

Zoloft vs lexapro: The lowest dosage form for Zoloft is 25 mg, the lowest dose to start with is 50 mg, but what for? Indications differ, and I wouldn't recommend doing it on your own, switching psychiatric medications without medical supervision is hazardous, kindly see or talk to your prescriber or seek a second opinion in person
