Tetanus Shot: A series of shots are given during childhood, but thereafter a booster shot is needed every 10 years, but as a practical matter, when I see people in the urgent care center with wounds or punctures, I recommend a booster if they cannot remember when they last had a booster. Better to be safe than sorry.

Depends: The best answer is it depends on lifestyle. For the average person at 11, the coverage should be effective for 10 years if the child completed the primary series earlier. If the kid likes to play football, walk in muddy streams, camps, hunts, works in the dirt a lot, i'd only count on 5 years before getting another booster.

Depends: If you can't remember it is likely  5 years since your last shot. If activities like gardening expose you to spores you should consider a booster. Diptheria deserves an update too. If your life ever brings you in contact with infants, you should consider the tdap which adds coverage for pertussis, which adults can get even if immunized as babies. This would prevent you from passing it to a baby.

It's time!: If you have an injury requiring a tetanus shot  repeat if it has been over 5 years.  Otherwise we repeat every 10 years.

Maybe: You should be protected from your past booster (10 yrs) unless the wound was contaminated or care delayed. Since you are at the end of the useful span of protection, a booster now or at your convenience might be a good idea.

By the way..Clean that wound out thoroughly with an antibacterial soap and water. Some folks prefer to use alcohol (it burns!), peroxide, or other "cleansing compounds" ( like povidone-iodine).
