Core and legs: Core strength and legs should now be part of the workout. Some of us would still have a specific body conformation and that might explain a specific body habitus.

: I would join a gum and ask for instructions on how to build up arms and chest muscles. Then do them and see if that helps.   Also look at the upper bodies of all of your blood relatives and see what they look like. This sort of body habitus likely runs in some families and gallops in others.

Weight Training: The best way to gain upper body weight as in muscle mass is in weight training the upper body.  Working the Biceps, triceps, deltoids, pectoralis muscles.  There is no diet that would help since the calories that you consume would naturally go to your fat stores.

Need fewer calories: Anything that uses energy (calories) will help one lose weight, as long as she does not eat or drink back more calories after the activity. Examples: if walking, take a longer route; use stairs instead of elevators; park a block from school and walk there with the kids; at home, skip dessert and go walk around the block instead. Doctor can check to be sure weight gain is not water, tumor, etc...

Weight gain: Is it just "weight" (including fat) that you're wanting to gain in your upper body, or are you looking more for muscle?  Gaining muscle requires a healthy food plan along with weight training to increase your muscles' size.  You will also need extra protein. See your doctor to make sure this won't be a risk to you, and so you stay healthy.  A trainer can help with workout guidance & diet too.

Weight training: You can build muscle mass in that area of the body by engaging in a systematic program of increased weigh training for the upper body. Best to start with a trainer. Avoid protein shakes, but eat healthy proteins, from nuts and beans, after each work-out.

Get checked!: Such a marked decreasing exercise tolerance is not normal.  Hard to say what's going on. See your doc for a thorough history and physical examination with appropriate testing.  Get checked please!  The sooner you find any problem the easier it is to treat!
