20 year old, healthy, rhr of 50-65, was diagnosed  this week with left ventricular high voltage (ecg). last year ecg was normal. is it normal?

Could be: Without more history, exam and having tests to review it is hard to be definite.An athelete can have the rate and increased left ventricular voltage. Athletic endeavors can cause an increased left ventricle  muscle mas and slow resting rate. "Increased" is a relative term and does not necessarily mean abnormal.

: AN EKG is an electrical shadow of the heart. It is not the entire picture. Ask your doctor who can see the entire picture of your heath

Normal: It can be perfectly normal. The classic pathological cause is elevated potassium but this would generally only occur in renal failure or overdose of a supplement. In people with acute chest pain, the very earliest sign of a heart attack can be a brief and temporary "hyperacute" peaking of the t wave. For most, though, it's a normal finding.

i've been experience high fast heartrates on and off since july. can sometime go month without any then back. ecgs are always ok. been sent to a cardio awaiting that appointment. it can be very high then down in a matter of mins again or last hours?

: It is not feasible to provide an opinion without additional history, physical examination may be some tests. It would be prudent to see your doctor to see if you may be depressed. Depression is treatable. Counter-intuitive as it may seem, exercise may help. Start with low intensity exercise such as walking and gradually increase the intensity as tolerated. Drink more water. Can be: Hypercalcemia can be evident on ekgs.  The most common wave morphology change is tall peaked t waves but there can also be wide qrs waves and flattened p waves.

Maybe, or not: Could be external influences (caffeine, stress, other meds/supplements, etc.), or internal (fast metabolism, body shape, etc.). Also, not everyone with hr=55 is a marathoner, and not all marathoners have hr=55. Should ask your dr at next regular visit for once-over and opinion, though.

Please clarify quest: The ECG may show either "tall" or "peaked" t waves, or may show "elevated" st segments. The description of "elevated t waves" is incorrect. Elevated st segments can mean many things, but at age 37 without any symptoms, are probably benign. Peaked t waves can also reflect a variety of issues, or may be benign. Would need to see the patient and the ECG to know for sure.

Many causes: Elevated t-waves can have a variety of causes. Heart conditions, such as poor blood flow through coronary arteries, can cause elevation. So can abnormalities of different chemistries in your body, for example, an elevated potasium level. Many of these things have many causes, so you need to work with your doctor to first find out what abnormality might be causing it, and then why you have that.

Different things: A short pr interval means that the heart beat is either initiated in a part of the atrium which is distinct from the sinus node (where it usually starts) or is being conducted with accelerated velocity to the ventricles. The other changes are "non-specific" and can be due to anything, like sodium, potassium or magnesium levels, medicines, lead position, skeletal variation and many other things.
