Symptoms of GERD: A hiatal hernia (hh) by itself rarely causes symptoms. Pain and discomfort are due to the upward flow of stomach acid, air, or bile.  When symptoms of a hh are seen, they include chest pain, which can mimic angina pain, a dry productive cough, usually at night, heartburn, worse when bending over and made better by sitting or standing up and difficulty in swallowing.

Yes and no: if the hiatal hernia is large enough to cause symptoms the it will not be pain LLQ it could cause heartburn,epigastric and chest pain from spasm
pain LLQ could be colon or spleen but more commonly it could be the gallbladder even if the pain is on the left not the right side of the abdomen

: Depending on how large the opening is in the diaphagm (the "hiatal hernia" opening) any abdominal pressure or compression of abdominal contents can enlarge the hernia.  Get a referral to a general surgeon to determine whether remedial surgery may be necessary.

Hiatal hernia GERD: First line therapy for hiatal hernia and gerd, (acid/bile reflux) is medications to lower the acid. If you fail medical therapy, then surgery may help you. Damaged/scarred lower esophagus and cancer can occur with NO therapy. Okay to ask your regular doc about medical therapy. A scope/EGD will be helpful to diagnose and help plan future therapies. Be cautious about surgery before medication.

One option: Alexis, surgery is an option, however you must weigh the risks and potential complications of undergoing surgery before proceeding. A 2cm hiatal hernia can certainly cause problems with eating, you certainly need to rule out other GI issues before having an operation that is irreversible. Continue working with your GI doctor, or ask for a referral to an esophageal specialist first to be sure.
