i have mottled skin on my lower back, left side, about the size of my hand. no pain, itching, swelling. has been there for about a month. ideas?

Mottled skin: You may have an ecchymosis - a spot of bleeding under the skin with diameter larger than 1 cm . It is similar to hematoma, commonly called a bruise. Most commonly, it coould be harmless caused by unnoticed trauma  but may be harbinger of more serious diseases such as purpura, and hemophilia. If it doesn't resolve or you get more eruptions see your doctor.

Specialist: Severe hand swelling and pain needs to be checked out.  If your family doctor or er has not been helpful you may need to see a specialists.  A rheumatologist or arthritis doctor may be best, but you usually have to wait several months to see them.  A hand surgeon could also be helpful.

Lots of things: Most important, if the pulses are not as strong as normal on both sides of the involved wrist, or the hand is very blue, seek immediate care (vascular emergency).  Someone else can give you a full list of possible causes.

Neurontin/hand pain : There are many reasons for hand pain. If there is suspected nerve problems causing the pain, Neurontin (gabapentin) may help. Swelling will not be helped and Neurontin (gabapentin) can actually cause swelling in the hands, legs and feet in some people.

Likely need Doctor v: Painless swelling in any part of the body requires an examination by a healthcare worker, a nurse or a doctor especially if the swelling is increasing over a period of 1-3 months. We also need to know how much/ what is the size of this swelling compared to your other/healthy Thumb? So depending on the above 2 factors you should get a doctor to examine your hand/Thumb in one or two months.

As long as there are: No shooting pains or weakness in your legs it is likely a pulled or strained muscle. Anti-inflammatory meds like Ibuprofen or Naproxen can help. Take as directed. The swelling more concerning. If pain worsens or same intensity last more than 2 weeks should consult orthopedic surgeon for exam and mri.  Can take 6 weeks to completely heal.

See your doctor: Pain swelling and redness are not the normal results of execs. They may be warning or symptoms of more significant problems.

mri and xrays are not showing any fractures nor swelling. what can be causing my hand pain after a fall four years ago when i landed on the fleshy part of my hand underneath my thumb?

I doubt if the current pain has anything to do with the fall years ago unless this pain has persisted in the last 4 years.

Injury: Even without bruising, you have sustained injury to that part of your body.  If the problem persists for more than a couple of weeks or worsens, consult your doctor.  In the meanwhile, try NSAIDs and ice .
