For good health - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form.  Drink enough water daily so that your urine is mostly colorless.
Practice safe sex.

CKD and transplant: Your question is difficult to answer. You will not have a transplant until you start dialysis, chronic kidney disease stage 6 (ckd), so you are not a candidate for one now. The results of a kidney transplant depend how well the kidney is a match and how well the anti-rejection drugs will work for  you. The answers to your questions about life expectancy and dialysis are too speculative to answer.

my 92. yr old dad in starting diuretic  now with 44 %ejection fraction and 3rd stage kidney disease. will he maybe get better using diuretics?

No: Diuretics treat one of the symptoms of low ef but do not improve the ef itself because they don't improve the mechanics of the heart. 44% is right on the border between mild and moderate dysfunction. The kidney disease has a bigger role in his overall health, but again, he's 92; all things considered, not too bad for a 10th decade of life.

Elderly pt: In elderly patients impacted stools is common and had many reasons. It doesn't reflect that the kidneys caused it or will worsen it. You Mentioned CKD stage III. Ensure she has adequate intake given summer months. She may need her kidney function followed. Best of luck. Patients at this age are fragile and benefit from all the help and care they can get.

: Stage 4 kidney disease mostly depends the age of the patient and the other medical problems of the patient. The Stage 4 kidney disease can be stable for a long time if the patient takes care of themselves. An 87 year old may have other medical problems that limit their life however. Just for perspective, based on age, the life expectancy of an 87 year old in general is about 5-6 years.

CHF and kidney dz: The combination is not a good one. Having said that predicting longevity is difficult except when cardiac function is so bad that body perfusion suggests it will be short.

OK: Renal tubular acidosis  (RTA) happens when there is impaired reabsorption of bicarbonate and/or impaired acidification by the kidneys. One way to treat RTA is to give alkaline substances (sodium bicarbonate). Cucumbers are considered high alkaline forming foods and should be ok to consume if you have rta.

Depends: His creatinine level is slightly elevated for his age.  It could also be related to dehydration.  Repeat labs and correction with other values may be helpful.
