Gestational Sac: The gestational sac is the first ultrasound sign of pregnancy.  You will usually see it beginning 5 weeks from your last menstrual period.  By 6 weeks you should see the yolk sac and by 6 and 1/2 to 7 weeks the embryo.

my fetus is measuring 7 weeks but gestational sac is 1 week behind. is that bad? heartbeat is 140bpm. is there anything i can do to help it grow?

Normal pregnancy: Just relax and don't worry . You will probably be fine. Worrying makes matter worse. Time will tell you. Subsequent us will  give better idea in second trimester.

Unclear: it may be too early to see a fetus and for sure it is too early for the heartbeat.
OR it could mean that the fetus has not developed and that the pregnancy will not develop.
no harm in waiting 10 days and repeating the ultrasound.
would advise blood pregnancy tests 3 days apart twice to see if levels rising appropriately also.

Most likely: The most accurate measurement of the fetus is the crown rump length. The gestational sac is like a fluid filled balloon, not a perfect sphere. So the measurement of the sac can vary quite a bit depending on what part was measured. If there is already a fetal heart beat, that is a great sign. Keep your follow up appointments with your OB doctor.

Heartbeat: If your sonogram confirmed an intrauterine pregnancy with a heartbeat then those are encouraging signs that things are progressing normally. A slight variation is the gestational sac size is less significant

See doctor: It is a matter of concern -,may be chromosomal abnormality or with the formation of sac .Your doctor will keep a check on subsequent  visit and on U/S finding about the growth of sac and fetus .

Maybe,miscarriage is: More likely. Not knowing how many weeks pregnant you are, it's difficult to give you more specific information. Usually the above consistent with early pregnancy loss. It is best to follow closely with Gyn/OB. Also try to avoid air travel or long car rides, trips in remote areas until this is settled. Miscarriage is potentially serious and can result in significant blood loss, so call 911 4 help

Doubtful: however all pregnancies carry risk of miscarriage, usually related to "blighted ovum" ( impaired egg) or genetic aberrations.
