can you help me understand criterion for gestalt therapy?

Gestalt Therapy: As a psychologist formally trained in the use of gestalt therapy, i can tell you that there is no criteria for you to see a gestalt therapist.  Each therapist however will have his/her own style and use of gestalt therapy,  essentially, you will be helped to become more aware of yourself, to live more in the present, and to explore the polarities that exist within you that might be hindering you.

Online search: One place to look online is "psychology today".  You can look up profiles of therapists who list their credentials and type of therapy.  There are no specific requirements for you to participate.  The therapist will likely conduct an intake or assessment to see if you are a good candidate.

Yes: Gestalt gives a set of 'techniques' and a therapeutic stance that can add a lot to one's practice, even if one is not primarily a gestalt therapist.

Not for everyone...: Gestalt therapy is considered a phenomenological-existential therapy that focuses on bringing immediate present experience into greater clarity and increasing awareness. The goal is for patients to become aware of what they are doing and how they are doing it, while learning to accept and esteem themselves. It's an excellent approach for less severe problems with effective functioning.

i was abused as a child and held captive in our home. now i obsessively ask my husband questions. is this related? don't want to relive in therapy. :(

It likely is related: While working out the early experience in therapy might be helpful, there are other ways to control this kind of obsessive behavior that do not require that kind of exploration. See a behavioral or cognitive behavioral therapist who can help you control the behavior without going into the terrible past.

what are good discussion questions for family therapy?

Ask about the dyads: Any question pertaining to the nature of the dyads in the family (one person to another) can be  productive towards deepening the family therapy process. An example: "jimmy, tell us about how you and your mother get along these days."

also, children can be extremely perceptive about the sources of family tension, so  asking the kid(s) about their views on the presenting issues can be useful.

Depends: Rules on insurance differ between states as well as insurance carriers.  Workman's comp will often cover pt if the injury was on the job, but this depends on your plan.

The : The physical therapist (pt) treating you can discharge you from therapy if he/she feels you have reached your treatment goals or feels that you are not responding to physical therapy. The doctor can request another physical therapy evaluation or speak to your therapist directly and get a better understanding of why you were discharged. They may be able to come up with a treatment plan together; if not your doctor can always refer you to another pt if they feel it is in your best interest. 
You should bring up all your concerns (expecially regarding your workers comp case) with your doctor or social worker if applicable.
