Neither: Reflux is caused when the acidic stomach contents backwash up the swallowing tube. It is common & the equivalent of a super wet burp.It is aggravated by infants pattern of filling up and laying down, where gravity is not available to help keep the feeds down. If it produces symptoms, acid reducers are often used to make the backwash less irritating.

Unusual: Some people are prone to ulcers because of the way their stomach oversecretes, and possibly abnormalities histologically, but acidic foods should not cause peptic ulcer formation

: The causes of peptic ulcer (either in the upper small intestine or the stomach) are unknown except Helicobacter pylori. You definitely should raise that possibility with your doctor; if H pylori is the cause, antibiotics are the main treatment. Without HP, acid control is the main plan. Correct that NSAIDs can be responsible, but in other patients there is no known cause.

PH monitoring probe: Gastro doctors can insert a device called a Bravo probe that measures stomach acid for 24+ hrs. (In the old days, a pH monitoring probe threaded through the nose did the same thing). An endoscopy only confirms the effects of excess stomach acid but doesn't measure acidity directly. Alkaline stomachs can cause heartburn. PPIs are newer meds, so long-term safety is unknown but appear to be safe 4now

R/O  GERD: GERD SX can be vague NAUEA/BLOATING /HEART BURN. Or Severe PAIN//REFLUX/LARYNGITS/COUGH You need a history and physical. Labs including Hpylori Ab Ig G and M. Stool for hpylori  x 3. I usually start with Zantac (ranitidine) 300mg. If symptoms have been longer than 8-10 weeks an endoscopy is warranted, while work up is in progress , stick to small frequent meals and gluten free diet.
