Unusual: Normally for biliary dyskinesia the ejection fraction should be less than 35%. The fact that the injection of cck reproduced your pain is interesting. A lot depends on how severe the pain is and working with your doctor surgeon to assess the best solution.

Surgery: If you have an abnormally low ejection fraction on a hida scan. And sludge  in gallbladder on ultrasound then you will likely have repeated bouts of pain and inflammation in upper abdomen. Best course of action is to have gallbladder removed laparoscopically.

Depends: On what you ate asking. Surgery can be done anytime. It depends on if the surgery will be of any help to you. Removing a normally functioning gallbladder can cause a lot of digestion problems. If a gallbladder functions less than 30-35% with associated gb pains, then there may be a benefit of gallbladder surgery. But not all patients improve with surgery either.

As a GI doc,...: Who orders and interprets a lot of hida scan results, you certainly could have symptomatic gallbladder disease. You shpould return to your GI doc or surgeon if your symptoms persist.

Weakened heart: The ejection fraction is the percentage of blood ejected with each heartbeat.  A normal ejection fraction would be greater than 50%.  When the heart is weakened for any reason, the ejection fraction falls.  There may be 100 different things that can weaken the heart.  The most common is coronary artery disease.

Gallbladder not work: That mean your gallbladder is not working as good as it suppose to be 
if you have symptoms of gallbladder colic consider medical treatment first if it did not work based on your symptoms make sure you do not have other upper digestive problems like ulcer or reflux or irritable bowel syndrome 
your last solution should be surgery do not jump to surgery too fast your doctor will advise you.

Poor heart function!: Ejection fraction is a measure of the pumping ability of the heart, and a low ef refers to suboptimal pumping ability in a diseased or damaged heart. The lower the number, the worse the heart function or capability. Several cardiac or pulmonary diagnoses can have low ef; talk to your physician or cardiologist for specific information about your heart and its condition.

Fasting: How severe will your fasting be? Hours,day?  Many patients on carvedolol fast 12 hours for lab work without problems. I and many physicians don't approve of severe fasting to the point of causing metabolic acidosis and are not fans of fad diets for anyone.

Chest pain: Chest pain can be from heart disease , inflamation of the lining of the lungs (pleurisy) caused by infection (pneumonia, etc).  Pain may also originate from the joints between the breast-bone and ribs. Indigestion, gall-bladder disease, stomach ulcers and acid reflux into the food pipe may cause chest pain also. Breast pain may be related to hormone changes or fibrocystic breast disease.
