Weakened heart: The ejection fraction is the percentage of blood ejected with each heartbeat. A normal ejection fraction would be greater than 50%. When the heart is weakened for any reason, the ejection fraction falls. There may be 100 different things that can weaken the heart. The most common is coronary artery disease.

Heart pumping poorly: A low ejection fraction means that the heart, particularly the left ventricle is pumping out only a small portion of the the blood that goes into it. Usually an ejection fraction of >70% is considered normal, while less than 70% means some degree of low ejection fraction, which will affect your exercise capacity proportionatelly.
