Get a second opinion: Such cytogenetic abnormalities in cll means that your disease may not stay still under watchful waiting. I would suggest that you seek counseling from an expert in cll. These folks are few and far between. So you will need to consider going to a major cancer center that deals with cll.

Probably not: With "good risk." it will fluctuate and the important thing is trend over time. Stay with the plan to monitor the CBC as recommended.

: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Talk to the doctor who ordered the tests. Having said that, what is your total lymphocyte count Smudge cells could be due to faulty technique in making a smear.

Can&#x27;t be answered: How many healthy people get smudge cells depends mostly on how rough the technician is while preparing the smear, and how eager to call a few broken lymphocytes &quot;smudge cells.&quot; if your absolute lymphocyte count is not way-high and you&#x27;re not anemic and feel well, stop worrying.

Yes.: Smudge cells are cells that are extremely fragile and easy to disrupt the cell membrane when making a blood smear.  Therefore, if the cell is disrupted it can be very hard to identify what kind of cell it is.  The most common and well known disease with smudge cells is cll.  However, many other hematologic diseases also have smudge cells.  Further testing is needed to be sure what cells are there.

i was diagnosed with cll 10 years ago, two years ago my white cell count rose to 84.  over the last 2 years it has come down to 54.  what does this mean?

Need information: In CLL, there is an overgrowth of lymphocytes which do not function normally against infection.  Normal counts are 4 to 11 K/mcl, so both 84 and 54 are very elevated.  The drop in count could suggest that the disease is improved, but one needs to also consider the anemia and platelet counts, a person's energy level, weight loss, night sweats,  and signs of infection to draw conclusion.

Not of diagnostic va: Not of clear value. If your CBC is otherwise normal then smudge cells can be ignored. By themselves alone they are do not imply a diagnosis of cll or all. Tell us the results of your CBC and differential counts so that we can be more accurate in informing you what it all means.

my wife just had an exam and had a wbc of 16, 000 with numerous smudge cells.  she is 47 and had no swelling of any kind.  is this necessarily cll?

It can be: You should definitely discuss this with your wife and with her doctor.  Usually smudge cells are indicative of something like a leukemia and warrant follow up with her doctor and probably a blood specialist.

Consult: Nowadays, every leukemia is typed by flow cytometry. Get with the laboratory and a hematologist who's at a major center. Hairy cell is extremely manageable nowadays.

Relatively lower.: Hematopoetic stem cell transplant (hsct) is done a atherapy for blood diseases like cll.  This is not always done because there are risks and complications.  The mortality is relatively high in the recipient - 10% or higher.  Thus these are used in life threatening conditions.  Complications include veno-occlusive disease, mucositis, infections (sepsis), graft-versus-host disease, and oral cancer.
