Knee effusion : Knee effusions can be from inflammatory arthritis, trauma w/who meniscus tears, hemarthrosis but what is surprising to me is that you have this for 10 years!. See a rheumatologist that can aspirate your fluid and analysis can show for example inflammation. Also blood work can ruled out Lyme disease for example, do it now since large amount of fluid is not normal.

Ankle inversion: Resting while elevating the injured ankle above heart level, compression such as with an ace wrap or preferably a more secure ankle splint, and ice usually help. Time is the limiting factor in most cases. Aspiration may be tried in severe cases but most of the fluid will return unless due to traumatic bleeding into the joint.

Several options!: When someone has post-traumatic swelling of the knee, i aspirate it immediately.  If there is pure blood, then some internal structure is almost always damaged, i get an mri-if positive, refer to orthopedics. If clear, inject with steroids and wrap.  I see no harm in waiting one week for your appointment.

Toenail pain: Four weeks after a partial toenail removal most patients will not be having any pain.  Most will not have swelling or redness in the area either.  If a patient has a lower immune system (Diabetic, etc), their healing rate will be much longer.  Check with the doc who removed the toenail and if your questions aren't answered don't be afraid to get a 2nd opinion! Best wishes!

i am concerned about a deep abrasion injury on my big toe getting infected. i have been washing with warm water/salt and dressing with supiroban and wound powder. toe is swollen but no redness/heat. wound oozes a clear fluid.

Abrasion: If no redness and toe is not oozing, this sounds that there is not yet infection. The toe will become increasing painful if infection starts. You can apply Silbecor to help with healing in stead of Supiroban. Supiroban, helps to prevent infection but can prolong healing.

Tenosynovitis: Is fluid in the tendon sheath.  You ankle has many tendons.  Ask your doctor what should be done for it.  There are various treatments.  Physical therapy. Anti inflammatory agents.
