Depends: But if truly a P wave, it depends on how pointy, as in height.  P waves with an amplitude greater than 2.5 small boxes in lead 2 or 1.5 boxes in lead V1 suggest right atrial enlargement.  If you've had an echocardiogram(sound study of the heart) like is mentioned in your "Procedures" then this would be more visible on this study.  It may not mean anything.

?: I suspect you're describing 2 dropped p waves. This is also called a sinus pause. It's usually due to increased vagal tone - a normal (autonomic) neurologic reflex. So long as you're not passing out or nearly passing out (I understand you're not &amp; I wouldn't expect you to), then it's perfectly within the range of a normal rhythm.

Exercise physiology: The heart rate normally accelerates with exercise, driven by faster sinus rhythm (indicated on an EKG by P waves.) There are occasionally P waves that come from other places than the sinus node (ectopic atrial rhythm.) It sounds like you've already had some testing. Not visibly. : There is usually atrial activity, either the occasional retrograde atrial activation, or just the normal sinus rate progressing along unaffected by the vf.   However, with the continuous ventricular activity in vf, it would typically be impossible to make out a P wave on the surface ekg.  When we have catheters in the heart (during an ep study) we see the atrial activity in vf.

P waves are atrial: "p" wave are a finding on the ECG (electrocardiogram) which represents atrial contraction.  During ventricular fibrillation there is no organized atrial activity and therefore there will be no P wave generated.

Ectopic atrial rhyth: You might have a ectopic atrial rhythm - which means your arial beats instead of originating in the sa node - originates else where. It is benign and nothing to worry about.  Get a standard 12 lead ekg to check.

Right atrial large: A tall P wave (or peaked or 2.5 mm in height) in lead II is consistent with possible RAE.  Right atrial enlargement. The gold standard diagnosis of RAE however would be echocardiogram or some other diagnostic test that visualises the heart, such as CT scan, MRI of heart, etc.
