Under certain circumstances, a fistula could develop an abscess or result in infection throughout the body, known as bacteremia or sepsis, all of which can be serious. Yes: You need an exam by a surgeon. Sometimes you may need an exam under anesthesia to correctly diagnose it and treat it at the same time. It could very well be an abscess which hasnt been treated properly.

See gastroenterology: Anal fistula in young man can be result of inflammatory bowel dz(  Crohn's dz).It is very important to establish diagnosis ( colonoscopy) to get prompt treatment .Dietary management also essential ( consume daily Probiotic rich food- Yogurt, Kefir) avoid processed, fry food, ref meat..) You need to see Gastroenterologist ASAP.
