Pain after injury: The vast majority of facial fractures happen after a trauma- car wreck, assault, fall.  You should have sought emergency care; thry should have examined with hands and maybe ct scan to look for these.  If it's far after the event, you may notice that the teeth don't fit together normally, there are changes in the coutours/rough edges when touching the face, there's pain or the eye(s) may sink in.

Depends: If nondisplaced fracture like anywhere else in body will heal without deformity. If depression or displacement can result in deformity. Yes depends  extent: Certainly, the cheek bone aka zygoma also supports the eyeball (globe). A fracture of the cheek bone with displacement can cause permanent deformity with a depressed or less prominent cheek bone as well as injury to the nerve to the mid cheek, nostirl and upper lip and can also cause vertical displacment of the globe with double vision & increased orbital volume with a sunken appearance to the eye.

A place to start looking for care could be at the university of oklahoma college of dentisrty, or a teaching hospital with an oral surgery residency. These teaching institutions may be able to take on your case at reduced or lower cost.

Also try to contact your county health department for guidance. The surgical repair will most likely involve a number of different medical specialties.
