Suspect bursitis.: People can have a bursitis with pain at end of range of motion. Usually slowly will get better. If not seen a doctor , please go and see on . Need films and maybe injection or PT.

have a radial head fracture by elbow. how come i have to have "aggressive range of motion" instead of a cast?

Avoid stiffness : Elbow is a complex joint that tends to get very stiff with prolonged immobilization so if the fracture of radial head  is not displaced and stable  then you only need  to rest  the elbow till  acute pain  subsides and then start range of motion exercises to prevent stiffness of the elbow good luck.

: Ask for proper  history and diagnostic exam. Tender over biceps tendon. Doing work such as shoveling before this started. Was shot in same arm or other arm?

Gentle exercise OK: Maintenance of range of motion is always a laudible goal as long as your pain is not made appreciably worse. Orthopaedic surgeon evaluatin will efficiently elucidate the underlying pathology. More common possibilities include: adhesive capsulitis, subacromial impingement (bursitis), a partial thickness rotator cuff tear, and/or underlying mild to moderate shoulder joint arthritis.

ROM exercises: Common range of motion (rom) exercises are heel slides, cycling, and wall slides.  These are started early in physical therapy.  More vigorous rom exercises would be things like stair lunges.  Most importantly, you should consult with a licensed physical therapist for a full evaluation to address your specific rom limitations and they will prescribe appropriate exercises for your particular case.

Strengthen muscles: You want to strengthen the surrounding agonist and antagonist muscles that are intimately involved with the ac joint: these would include but not restricted to the trapezius, deltoid, pectoralis major and scapular thoracic muscles including the levator scapula, rhomboids and serratus.

Because the symptoms can be so varied (TMJ, Chronic fatigue, joint/muscle pain, osteoarthritis, headaches) it can mimic many other disorders. Finding a good specialist who can do a thorough evaluation is vital for persons who suspect hEDS.

After 6 years,: you've likely maxed out on what you'll get out of having the procedure performed.  With most surgical procedures, full benefits are obtained after 12-14 months.  It all depends on  exactly what was done, the therapy ordered and how hard you worked at therapy.  I'd be more concerned with any remaining pain or disability than specifically the range of motion.
