However, accurate identification of the injury is important to develop the proper therapy plan. This is particularly important for more serious injuries. See you family doctor, orthopedist, or physiatrist for a referral to a physical therapist if necessary.

Inflammation: When a jont is injured, the lining of the joing is inflammed and the tendons, ligaments and muscles also get stressed and inflammed.  When restoration of function is attempted and motion is induced and stretching occurs, there is activation of pain fibers.  That is why a therapist is needed to guide you through this.  Medications and pain alleviating measures may be needed to accomplish end goal.

Depends: I am not sure what you mean by a dislocated knee. If it is indicated dislocated (the bones of the knee joint are not in the right position against each other), then they need to be manipulated back to what they were before the injury. Once they've been placed back in the correct position, immobilization is warranted using a brace or a similar device. To strengthen muscles/ligaments, may go to pt.

Careful: Depends on the ligament. It is very important to get your motion back first after a ligament injury. Range of motion exercises (wall slides, etc.) and biking without resistance can accomplish this. Then if you are treating the ligament nonop, strengthening exercises are begun. For example- PCL injury, the quad is very important.Ask your doctor for specific goals for your specific ligament injury.

Depends: What did your physical therapist or physician prescribe?  Best to follow the home exercise program prescribed.  However, strengthening exercises (with weights or just using your body weight) are every other day (allowing a day for rest and recovery), so 3 days per week.  You should stretch daily though holding each stretch for 30 sec - use your watch as tendency to think your done in 15 or 20 sec.

Impossible to know: Without more detail, it's not possible to tell you why your knee is not improving. Knee conditions arise from a variety of reasons, and we would need an understanding of the cause of your knee issue, an examination of your knee, and possibly an x-ray.

It is best to make an appointment with your primary care provider for further evaluation, treatment or referral to a specialist.

Biking: Biking is a great way to help regain range of motion and strength after ACL reconstruction.  Later in the rehab process, lunges and squats would be appropriate.  But it is important to progress slowly and with proper form.  I highly recommend that you seek guidance from a physical therapist who can help with your exercise progression and technique.

See an Orthopedist: Swelling and hyperextension could indicate a ligament tear, most likely the acl. You could have also torn cartilage (meniscus) which could also cause your symptoms.   See an orthopedic surgeon who can examine the knee and determine if you need x-ray, MRI or are ok to treat conservatively (rest, compression, ice, therapy).  Many orthopedic offices have a same day clinic for acute injuries.
