Unsure about what: Ed means. If you mean emergency dept. Then there is really no need to go to "ed". Go to a primary physician who will then order a chest xray and ct scan of the chest for evaluation. Your clinical findings may indicate myasthenia gravis or thymoma.

Yes: As much as possible, thymus tumor should be removed. For any residual thymus that cannot be removed, radiotherapy or chemotherapy has to be considered.

Thoracic Surgeon.: An "enlarged" thymus gland "attached to breast bone" with chest pain and shortness of breath, needs to be evaluated by an endocrinologist and possibly by a thoracic surgeon, the sooner the better.

Lymph node: Most cases are not serious but if your lymph gland doesn't go away in six weeks, discuss it with your doctor. Sometimes serious problems are present and express themself this way.

Perhaps: More likely the described lymph node enlargement is secondary to a dental or tonsillar infection, but does indeed need attention.  Suggest see your regular doctor and your dentist.

is it unusual for a left-sided submanibular gland to be quite visibly enlarged for over a month (with no other symptoms) and for an ultrasound to detect no abnormality in the neck at all?  i'm baffled.  any suggestions or advice, please?

Otolaryngologist: The submandibular gland enlargement may be caused by a silent blockage in the duct by a stone. An Otolaryngologist is best qualified to evaluate,advise and treat you. I assume that this is not a pathologic lymph gland that should be biopsied.

Enlarged gland: If on the neck it is not a salivary gland.  It's most probably a lymph gland.  Should be seen immediately to discover cause and appropriate cure.  If a lymph gland, yes, lymphoma is a possibility.  See your GP or an ENT for differential diagnosis.

Lymph nodes: enlargement of local lymph nodes may be due to local infection and may get better with the use of antibiotics. please see a physician,  you may have a simple easily treatable problem like local infection
