Fatty liver to start: Distinguish fatty liver (steatosis) from fatty liver with abnormal liver tests (steatohepatitis). Best to start with a weight loss diet based on calorie and fat restriction, avoid alcohol, control diabetes &amp; cholesterol (if present).  Your doctor will likely want to monitor your labwork and imaging, &amp; consider all diseases that can promote steatosis.

Shouldn't: Cirrhosis causes splenomegaly through portal hypertension and splenic congestion; a mildly fatty liver should be well before this stage, and should not be the cause of a painful enlarged spleen.  You should see a doctor for further evaluation and testing.

In bad cases it can sometimes cause a type of cirrhosis. An enlarged spleen can be caused by significant liver damage from a variety of causes, or an enlarged spleen can be caused purely from spleen or blood disorders.

Splenomegaly: Yes, splenomegaly can cause referred pain to the left shoulder due to irritation of the adjacent diaphragm and the anatomic course of the associated phrenic nerve.  If the pain is acute, it can be a sign of splenic rupture(Kehr's sign), which can be spontaneous in the setting of slpenomegaly.  If this is the case, seek medical attention emergently.

can being obese cause an enlarged spleen?  i was severely overweight, have fatty liver, but lft's are normal. so the liver seems to be functioning fine, wondering what could cause the spleen issue?

Yes: Non alcoholic fatty liver can cause portal hypertension which is a cause of spleen enlargement. Portal hypertension means that there is increased pressure and abnormal blood flow in portal vein system. Portal vein brings blood to the liver.

More info needed: This depends entirely on the cause of the liver problem and whether it is progressing to cirrhosis (or might already be there). If the enlarged spleen is due to portal hypertension -- increased pressure in liver veins -- it could be pretty advanced. This is something to discuss with your doctor.

Ok: It is find as long as not used in excess. Remember that oil is oil.   You do need to find out why your liver and spleen are enlarged however. Have you seen any specialist yet?
