Can be mononucleosus: Monocytes become elevated when the immune system needs to produce more antibodies to handle the  intracellular invading agents.. Mononucleosis is caused by the epstein bar virus which can live within the nucleus and cytoplasm of the cell and suppresses the immune system, and can only be controlled by antibodies made by intracellular monocytes. The monocytes will be elevated as part of the WBC coun.

Not clear: The statement suggests the blood test is 4and one half years old.  I doubt that but if that were the case, the age of the test is meaningless.  However, if you are talking about a 4 and a half year old child. the symptoms, and blood test are consistent with a viral upper respiratory tract infection.  The iron deficiency is not necessarily related to the URI.

Follow-up: It is important to follow-up with pediatrician and follow the symptoms.  It is still probably a viral illness, but the longer the fever goes on the more the medical team needs to think about bacterial infection, or other possibilities including autoiimune disease like rheumatoid arthritis or mediterranean fever, or possibly oncological reason

Not sure.: I don't believe there is a connection here. I suspect you have collected some random data. What you really need to know is what symptoms he has and how these labs and other unmentioned labs compare to priors. His doctor should probably explain these to him.

ive had elevated monocytes for the last 3 months. ive went to a hematologist and she told me to not be concerned unless i experience symptoms. i wanted to ask if i should push for a bone marrow biopsy to rule out malignancy as thats my main concer?

: The best person to decide what tests you need is the hematologist that knows your full history and findings. Making a few vague statements online does not provide anyone with adequate information to change that fact. Share your worries with your doc, or get a second opinion.

i have consistently low wbc, 3.5& elevated monocyte 12.8, eosinophil 9.1, basophil 2.3, neutrophil 1.5. doctor seems unconcerned & blames it on "allergies" though ive never been diagnosed with, nor have any allergies sx
