Mutation: Anyone with duodenal adenocarcinoma deserves to be checked for the same genetic mutations that produce colon cancer. Or it may be dumb luck. I hope you have a good result from what i expect will be a surgical intervention.

The duodenum is the: 12 inches of small bowell after the gastric antrum.  The ducts from the liver and pancreas enter quickly and the remainder sweeps below/behind the pancreas.  The work up needs to determine the blood vessels (aorta, ivc, sma, smv, portal vein). The biopsy usually is deon endoscopically. The treatment is whipple, pancreatoduodenectomy, with roux-en-y loops to drain bile.

AdenoCa in mucosa: An uncommon lesion arising in the mucosa of the 1st to 4th portion of duodenum presenting with obstruction or bleeding.  In many cases lesion involves medial ampulla of Vater and obstructs pancreatic duct system requiring Whipple.  Prognosis here however better with resection than lesion arising in pancreatic ducts.

Nobody knows: I don't know what it is- it could be many things. If possible, a biopsy or surgical procedure to remove it should be done - especially if there are symptoms related to it- i.e. Bleeding etc.

i had egd shows localized nodular mucosa duodenal bulb and hyperemic gastric body.i also have an adrenal tumor on right side. is this cancer?

EDG findings: Adrenal tumors are often benign, ct/ultrasound is often beneficial in the determination.

Your egd findings suggest gastritis (erythema) , and nodular duodenitis.  Both are usually benign in your age group.  Occasionally a biopsy of these areas may warrant repeat endoscopic evaluation in the future if the pathology suggests tissue changes that could be premalignant.

my girlfriend cant keep hydrated orally. has been on iv drip for 4 months. what can the problem be. she had a colon resection. is not getting better. no cancer found. endoscopy revealed ulcers in duodenum.

See GI: It does not make any sense to me.  Is she losing water because of excessive urination?  If so, one needs to consider diabetes insipidus assuming that uncontrolled diabetes mellitus has been ruled out.  The colon resection (why?) and the duodenal ulcers have nothing to do with hydration.  The latter needs treatment also. 
Check with your internist to see if referral to an university center is neede

Pancreatitis: For acute pancreatitis you would need your lipase to upward of three times the normal limit and/or have CT image evidence with epigastric pain. Elevated lipase can be non-specific and can be retested   Pancreatic Ca unlikely at your age though CT would definitely help in viewing any possible tumors there. Chonic lipase elevatation can be due to chronic pancreatitis and further testing should be.

are there any early indicatiors if the thickening of the duodenum is from infection vs cancer or food allergy? or wait for histology from biopsy?

No: There is no good way to tell for sure without a biopsy but I can tell you that food allergy is not expected to cause duodenal thickening.

why do esophageal cancer patients have a feeding tube inserted into the duodenum instead of the stomach?

Surgery location: Short answer from a non-surgeon.
The percutaneous endoscopic gastrostomy (peg) feeding tube may interfere with a potential esophageal surgery more than a percutaneous endoscopic jejunum (or duodenum) (pej) tube in the small bowel (after the stomach). This isn't always the case, but one reason.
