Similar: They are very similar (as are all opioids).  Obviously a transdermal product (fentanyl patch) is much easier to use than having to inject something iv.  In my experience the incidence of nausea, vomiting and constipation are better with the fentayl patch.  Dilaudid is available as both an immediate release oral product and an extended release product (exalgo).

No: Generally, unless you get some sweating as a side effect there would not be any point in taking any opioid for a fever. Tylenol (acetaminophen) or any of the nsaid's are effective at reducing fever. If there isn't any response you need to seek medical attention.

Do U mean stage 4?: Stage 4 fibrosis, associated with hepatitis C, is cirrhosis. Talk frankly with your liver specialist about use of any drugs in this setting, particularly if your liver is decompensated. By the way, you haven't shared the nature or source of your pain, so this response cannot be better targeted to your question.

Pancreatitis : Make sure u only use dilaudid for acute bout of pain as tolerance to that drug could be an issue in the future secondly ask your gi doc for someone who does a celiac plexus block which can be helpful

Pain in the neck: what you need is a diagnosis and a program of physical therapy. Recent study showed that UNDERSTANDING the pain source helps with proper PT.

Your: Primary doctor will be in best position to answer the question. My guess is he saw no advantage in switching to dilaudid and or he wanted a longer acting with ease of administration

Strongest opiate: Fentanyl is the strongest followed by dilaudid, Percocet and narco. However, over time every opiate produces tolerance and withdrawals if stopped abruptly.  They are usually never a long term solution for chronic non cancer pain. Fentanyl hsbc a black box warning to be used only for cancer pain in a patch form. It is used IV for intra operative pain during surgery.

Pain meds: Hopefully you are doing this under your doctors supervision. That being said, continue to follow your doctors supervision to decrease your medications as we have no idea what you are being treated for or what dose of the medication you are on.

Yes: That is a very large amount of dilaudid. Dilaudid is a powerful narcotic and opioid naive patients will normally start around 1-2 mg. I am not sure why you are asking, but please do not take this large of a dose and if you have and you do not normally take opioids, please have someone take you to the er. Otherwise, a 20mg dose at once should be avoided. I hope this answer helps.

Narcotic: Fentanyl is very potent synthetic analgetic, known under many commercial names:  sublimaze, actiq, durogesic, duragesic, (fentanyl) fentora, matrifen, haldid, onsolis, and a few others. It is highly lipophilic, which increases its potency. Fentanyl binds ?-opioid g-protein-coupled receptors, which inhibit pain neurotransmitter release. Side effects: respiratory depression, chest rigidity.
