Anti-acid therapy: Sounds like you need some antacid therapy to start. This should help both the gerd symptoms and the gastritis.  Make sure to ask if you have H. Pylori infection as a cause of the gastritis. This needs to be treated with antibiotics as well. Forget about the hiatal hernia unless it becomes very large. Hope this helps!

Need doc: Assuming these diagnoses were made with appropriate imaging and are correct, then you need to inform the treating doctor. "plan b" may be in order. Were you instructed to modify certain life style issues such as drinking alcohol and smoking and you didn't? The medications need help from the patient. Monitor your stool for black tarry stools which are a sign of bleeding in esophagus and/or stomach.

Meds are better : The conditions you describe are serious. Diet and over the counter meds may help some.  However, the prescription medications - antibiotics and acid lowering mediations are safe and effective. They will help you and have very low side effects. Serious complications can occur from what you have.  I'd recommend seeing a doctor again and do take the medications.

Gut: Although PPI therapy can be helpful short run, sometimes for the long run it can reduce stomach acid an cause problems with digestion. Gut problems typically need a multifactorial approach. Stay away from coffees, teas, soda, tobacco. Consider the followng: probiotics, bovine colostrum, digestive enzymes. Paradoxically some people actually need to take more acid especially when then eat.

Not GERD: Gerd typically causes heartburn and regurgitation. It can also cause inflammation in the esophagus, but not the stomach. Amtrak gastritis can have many causes, but gerd is not one of them. Call your GI doctor for clarification.

Not likely: Small bowel cancer is very rare. It may show up on a ct scan or a small bowel series x-ray. Usually presents as painless anemia or bowel obstruction.

The prefix "gastro" means "stomach."  the term "gastritis" thus means an "inflammed stomach."    the term is often used imprecisely to mean stomach upset, which can derive from both acid & non-acid causes, slow emptying of the stomach, h.Pylori infection, atrophic & hypersecretory problems.  Try acid blockers & diet modification, recommended by your doctor.
