Fordyce spots : Fordyce spots are sebaceous glands that are present in most individuals. They a are seen on the genitals and/or in the mouth as small, painless, raised, pale, red or white spots or bumps 1 to 3 mm in diameter that may appear on the scrotum, shaft of the penis or on the labia, and on the inner surfaceand vermilion border of the lips of the face. Herpes is very different with shallow ulcers.

Big difference: Fordyce spots (or granules) are natural findings in the oral cavity. Lip cancer looks vastly different. Your oral surgeon should have a look.

Pain: The biggest difference would be pain. Herpes in the oral cavity hurts, usually quite severely. Fordyce granules are always there and do not cause a problem. Herpes lesion develop from previous normal lining of the mouth. Frost e granules are always there.

Many possibilities: There are many things that could cause the skin to itch.  Several types of inflammatory conditions of the skin, allergic reactions, and sometimes bacterial, viral or fungal infections can cause rashes on the skin. Insect bites can also be the cause of a rash on the skin.  Have it evaluated by a professional and get the appropriate treatment.

Infected?: Depends what you mean by infected. If major pain and lip swelling, see an oral surgeon ASAP. If merely inflamed, should heal rapidly with OTC meds, Orajel, saline rinse.
If getting worse, see oral surgeon ASAP.

: Herpes is not possible, based on this description. No herpes lesion ever lasts more than a week or two. Maybe Fordyce spot, small cyst, fibroma, or something else.
