Not simple: Simple cysts have no echoes and complicated cysts have high level echoes. Low level echoes means cysts that are less complex but not simple. These cysts need more aggressive treatment and confirmation that they resolve.

Unsure answer: This is difficult to explain without the history as to why the first ultrasound was done and if normal why the second one was done then as well.  Also important whether period is active and still ovulating.  This second ultrasound shows a probable fluid in a cyst with separation but this appears in 3 months so may mean nothing but make sure follow as they are doing.  Need to consult with your Dr.

Complex cyst: These are often cysts with blood products within them. The lack of blood flow is reassuring that there is no evidence of cancer. If you have no symptoms ask your doctor for a followup ultrasound after 3 cycles.

Malignancy: In your age group (27 is correct, yes?) not terribly high for malignancy but not out of the question. benign tumor (non-cancer) or functional cyst is much more likely. Clinical history would be very helpful, blood testing sometimes too.  Very dubious that it would cause urinary frequency. Hope this help!

Ovarian cyst: Make sure the cyst is monitored for resolution, by your doctor, by repeating a sonogram in 2-3 months.  Cysts are fairly common and most do disappear, but new ones may or may not can form. Usually no bearing on one's fertility, unless you have polycystic ovarian syndrome.  You should know if you have this or not.

: At any age, the finding of an ovarian cyst with "internal septation, complexity with nodularity" requires biopsy.  See your GYN physician for an exam and treatment to be certain about the nature of this abnormality.
