i have a red hard swollen bump, with a black dot on the bump. it is very hot and swollen. pus is starting to come out now. day3

Boil or abcess: This abscess is draining pus loaded with bacteria, usually staph. Continue warm applications 4 times a day to continue drainage, but 
after soaks, sterilize the surrounding skin with rubbing alcohol and cover the abscess with a gauze held in place with paper tape
and not adhesive tape.

. See MD if persistent pain,swelling or spreading redness occurs.

i popped a cyst on foreskin, blood came out. now it grew twice in size and with a black dot.. went to a doctor he gave antibiotic but it is the same?

Cyst on foreskin: Sounds like you opened the cyst and it closed back up  - I would take the antibiotics and try warm compresses to the cyst.   It may open up again or if it doesn't it may need to be lanced to get the cyst contents out.

Exam: You need to pursue a full exam with an ophthalmologist if you have scotomas or scintillating scotomas and any area of visual defect or total loss of vision in one eye that is persistent then you should be seen emergently for a full medical history and physical exam including a comprehensive eye exam by a specialist. Anal lesion: Please let your parents know about your concerns. We don't know what is causing that black dot, so it would be hard to recommend how to treat it. An  evaluation would help to determine the cause and a treatment plan can be developed at that poing.

Sebaceous cyst...: ...is most likely. Think of it as a giant blackhead. Sometimes sebeceous cysts can become infected, like when a blackhead becomes a pimple. Whether this needs medical attention depends on size, location, and whether it gets infected from time to time (red, sore, pus drainage). If uncertain, your husband should see his GP. No hurry if not currently red, sore, etc. Good luck.

i have a very small, flat freckle near my waistline with an even smaller black dot in the middle. should i be concerned?

Freckle : Difficult to say since I don't know how long that black dot in middle has existed. Have your physician take a look. Usually - as long as there are no changes to any borders then usually not much concern.  However a trained provider must do a skin exam to determine if there another condition present.  Good luck
